This position will be responsible for, but not limited to, coordinating all work related to admissions; working closely with hospital discharge planners, Hospice and Home Health agencies, and Givens Estates Social Workers; working with Medicare and Insurance companies including verification of insurances; and ensuring that admissions are smooth and efficient; marketing Givens Estates Health Center to area hospitals, assisted living facilities, physician offices, etc.; communicating to team resident initial discharge plans and will work closely with facility Discharge planner; and working closely with Givens Estates residents.
JOB REQUIREMENTS: The Preferred Candidate must have exceptional communication skills, be well organized, be an energetic team player, self-starter, flexible and able to manage multiple tasks with little to no supervision. The Preferred candidate will have a Bachelor's degree in Social Work or equivalent degree and will have exceptional computer skills. Experience working in long term care is preferred.
